Our Patient Portal allows you to confirm or correct your personal information, emergency contact, insurance information, previous surgeries, reason for therapy and medication. 

Please help us out by filling out all of your information online, including all medications. We try to spend as much time in office with each patient, and it is very time consuming for us to enter each patient's medication list. Thank you!
